STEPS TO ENROLL

If you are seeking services for your child, follow these 3 easy steps:

Gather
Your Documents
If your child has been diagnosed with Autism Spectrum Disorder (ASD), gather the diagnostic report, your child's insurance information and a physician's referral for ABA Therapy. All will be required in order to access services.

Contact Cornerstone Autism Center
Our Intake Coordinator will take your information, verify insurance eligibility and walk you through our enrollment process. She can answer questions you may have about insurance, and will schedule an evaluation.

3

Participate in Evaluation and Onboarding
Your child will be evaluated by one of our Board Certified Behavior Analysts, while another staff member conducts a parent interview. All information gathered will be used to create a comprehensive Treatment Plan that is submitted to your insurance company for authorization. Once approved, onboarding will be scheduled and your child will begin at Cornerstone!
