ABA Therapy Application

If you think Cornerstone Autism Center might be an option for your child, you are encouraged to schedule an appointment to tour the center. You will be able to see the facilities and ask any questions you may have about ABA, our program or insurance.

If you are interested in having your child attend Cornerstone, complete and submit an Online Application and e-mail us the Insurance Registration form. You can fill out the online application below and e-mail your complete registration form to Intake Coordinator Vicki Moyer at vmoyer@cornerstoneautismcenter.com. Once we receive your application, we will research your insurance and schedule an evaluation with one of our BCBAs.

Enrollment Considerations


Child must be diagnosed with an autism spectrum disorder. In addition to a formal diagnosis, your child will need a Care Plan letter from his or her physician indicating that ABA Therapy is a recommended treatment.

Parents must provide own transportation to and from center. Therapy hours are Monday through Friday, 8:30 AM to 3:30 PM.

Source of funding must be secure before services begin. If your child is currently insured by a group or individual policy that qualifies under the Indiana Autism Mandate, please complete a Registration Form with the appropriate information. We will verify insurance coverage and provide a summary of your benefits. We are able to bill commercial insurance carriers such as Anthem, United Healthcare, Sagamore, Advantage Health, etc. Cornerstone is also a registered provider with Children’s Special Health Care Services Program. Cornerstone is also a Medicaid provider.

If your child has insurance that does not fall under the mandate, or you are unsure of your insurance coverage, please call our center and ask about your options. We have successfully counseled many families regarding their insurance coverage in regard to ABA therapy and we are happy to guide you through the process.
